Prepare Required Information

Prepare a Letter of Authorization

• Ensure it is in PDF format and does not exceed 4MB. Here’s a template you can use. Complete the provided porting form with the prepared information.

Copy of Recent Phone Billing Statement: 

• Must be a scanned or electronic copy in PDF format, not exceeding 4MB.
